Most expertise is noticed only when it fails.

When a bridge collapses, we notice engineers.

When a medication causes harm, we notice pharmacists.

When a project runs over budget, we notice planners.

When communication breaks down, we notice editors.

But when things work exactly as intended, the expertise behind them often disappears from view.

Success has a curious side effect: it hides the work that made success possible.

I spent much of my career as a pharmacist. Patients often thanked me for explaining a medication, helping them understand a side effect, or answering a question their physician didn’t have time to address. I appreciated those moments.

What patients rarely saw were the problems that never reached them.

They didn’t see the clarified prescription before it confused them.

They didn’t see the corrected dosage.

They didn’t see the potential interaction and resolve it before it could cause harm.

They didn’t see the phone call to a physician’s office or the quiet consultation with another healthcare professional.

In many cases, the best outcome was that nothing happened.

The patient received the right medication, took it safely, and went on with life.

The expertise had done its job so effectively that it became invisible.

I’ve come to realize that this phenomenon extends far beyond healthcare.

Editors remove mistakes before readers encounter them.

Engineers design systems that prevent failures.

Laboratory scientists generate data that guides decisions.

Grant writers translate complex ideas into proposals that secure funding for research, education, and community programs.

Teachers explain difficult concepts until they seem simple.

Leaders make countless decisions that prevent problems their teams never know existed.

The better these professionals perform their work, the less visible their contributions often become.

There is an irony in this.

We celebrate dramatic rescues, breakthrough discoveries, and visible achievements. Stories draw us with obvious heroes and clear turning points. Yet much of what allows modern society to function depends on people whose greatest accomplishments are measured by the absence of crisis.

A medication error prevented.

A structural failure avoided.

A misunderstanding clarified.

A problem solved before anyone realized it existed.

These successes rarely make headlines.

They rarely receive awards.

They often pass unnoticed.

Perhaps that is why it is easy to underestimate expertise.

When something appears effortless, we sometimes assume it was easy.

When a process runs smoothly, we forget the years of knowledge, training, and experience required to make it appear that way.

The musician makes the difficult passage sound natural.

The teacher makes the complex concept understandable.

The pharmacist makes the medication regimen manageable.

The writer makes the complicated idea clear.

What we witness is the result.

What we often miss is the expertise behind it.

As I’ve grown older, I’ve become increasingly interested in the hidden work that supports our lives. The people who explain, guide, anticipate, and protect—the individuals whose contributions disappear into the smooth functioning of the systems around us.

Their work reminds me that expertise is not merely the accumulation of knowledge.

It is the ability to apply knowledge in service of others.

And when practiced at its highest level, it often becomes nearly invisible.

Perhaps the true measure of expertise is not how often it attracts attention.

Perhaps it is how rarely it needs to.

And perhaps one of the most important things we can do is learn to notice the people whose work becomes invisible precisely because they do it so well.

The People Between the Sessions

I attended the Nonfiction Authors Association annual conference expecting to fill a notebook.

I did.

The speakers were excellent. The sessions were informative. I came away with pages of notes on publishing, marketing, speaking, and building an author platform.

But as the days passed, I realized the most valuable thing I brought home wasn’t in my notebook.

It was the people.

Like many writers, I attended hoping to learn. My book, Safe to Swallow, is moving steadily toward completion, and I know it’s time to take publishing and marketing more seriously. I wanted information. I wanted feedback. I wanted to understand what comes next.

What I didn’t expect was how much encouragement I would find.

One of the highlights of the conference was a breakout session where I was placed in a small group with several other writers, including Christina VandePohl, a physician and medical examiner writing about her career, and Mike Larsen, a retired literary agent, editor, and author.

On the surface, Christina and I came from different worlds. Yet we quickly discovered common ground. She writes from her experiences in medicine. I write from mine in pharmacy. She’s working on a memoir. I’m working on memoir and mystery fiction. Before long, we weren’t simply exchanging ideas — we were supporting one another.

A conference session had introduced us. A friendship was beginning to form.

That happened repeatedly throughout the conference.

Writers shared experiences. They exchanged websites and email addresses. They offered suggestions and encouragement. Nobody seemed guarded or competitive. People genuinely wanted one another to succeed.

I remember thinking: I’ve never been part of a group quite like this.

Not in business.

Not in healthcare.

Not even in church.

Everywhere I turned, people were generous with their time, knowledge, and support.

Perhaps what surprised me most was how naturally everyone accepted me as a writer.

During the sessions, I participated in the chat, shared ideas, answered questions, and learned from others. Nobody asked whether I was “really” a writer. Nobody questioned my credentials because I had spent most of my career as a pharmacist and healthcare professional.

To them, I was a writer.

Simple as that.

Sometimes we don’t realize how much we need others seeing us until it happens.

When the conference ended, I felt encouraged, energized, inspired, and grateful. But if I had to choose the two feelings that stayed with me the longest, they would be these:

Seen.

Validated.

I attended the conference looking for information.

I found that.

What I didn’t expect was to find community.

When people talk about conferences, they often focus on the keynote speakers, the workshops, and the presentations. Those things matter.

But sometimes the most important moments happen between the sessions.

A conversation.

A shared story.

A word of encouragement.

A new friendship.

A reminder that we aren’t making the journey alone.

Those are the moments I brought home with me.

And they never appeared in my notebook.

The Distance Between an Idea and a Laboratory

Innovation rarely succeeds on ideas alone. Ecosystems matter too.

At a recent life sciences networking event in Indianapolis, I found myself in conversation with a young commercial real estate professional named Luke. We were discussing one of the persistent challenges facing emerging biotech companies.

Lab space.

Not eventually.

Immediately.

A company may have the talent, the science, the investors, and the ambition. But if suitable laboratory space is not readily available, momentum slows. Opportunities drift elsewhere. Sometimes, entire companies relocate.

Boston came up repeatedly in our conversation.

Not because Boston necessarily has smarter scientists or better ideas, but because it has built something equally important:

readiness.

In places where innovation ecosystems mature over decades, the distance between an idea and a functioning laboratory becomes remarkably short.

That distance matters more than many people realize.

Innovation is often portrayed as a lightning strike — a brilliant insight suddenly changing the world. But in reality, innovation is usually an ecosystem.

It depends on:
research institutions,
investors,
physical infrastructure,
skilled workers,
suppliers,
transportation,
regulatory expertise,
and perhaps most importantly, proximity.

Ideas move faster when the people who can advance them are already nearby.

During our conversation, I found myself thinking about how often success is attributed entirely to talent, while the surrounding environment receives far less attention.

But environments matter.

Some environments accelerate possibility.

Others unintentionally place friction in its path.

Environments do matter; not only in biotechnology, but in nearly every area of life.

Writers need communities.

Artists need audiences.

Entrepreneurs need networks.

Patients need access.

Students need encouragement.

Even personal growth often depends on whether someone’s environment supports curiosity or quietly discourages it.

The longer I listened during the event, the more I realized that many people in healthcare, research, and business are not simply building companies.

They are trying to build ecosystems.

That is much harder.

Buildings alone are not ecosystems.

Neither are slogans.

Real ecosystems emerge slowly through trust, collaboration, repeated investment, and a shared belief that innovation is worth supporting long before success is guaranteed.

I left the event feeling encouraged by the number of people in Indiana genuinely trying to strengthen that foundation.

Not through hype.

Through persistence.

Through relationships.

Through long-term thinking.

And perhaps that is one of the quiet truths behind every thriving center of innovation.

The most successful ecosystems are not built merely on ambition.

They are built on readiness.

On shortening the distance between possibility and progress.

Why Human Explanation Matters More Than Ever in Healthcare

Normally, my Observations posts appear on Wednesdays.

This one could not wait.

This morning, I walked into a professional documentary studio for the first time in my life.

Like many people, I had only seen studios in photographs or films. The lighting. The cameras. The suspended microphones. The quiet choreography behind the scenes. Everything felt calm, focused, and intensely purposeful.

I arrived early due to road construction concerns in Indianapolis.

I came dressed in business casual.

The Harmacy team arrived relaxed in shorts and summer shirts.

Within minutes, none of that mattered.

What struck me immediately was not the equipment or the setting, but the atmosphere: graciousness, warmth, curiosity, and genuine listening.

Marie and Anaïs welcomed me not as a “subject,” but as a collaborator.

And something unexpected happened.

I realized I was not there to talk about pharmacy.

I was there to help translate it.

For years, I worked in pharmacy systems that were becoming increasingly complex, pressured, and fragmented. Patients are often left with prescriptions in hand but lingering uncertainty in their minds. Pharmacists understood far more than many people realized, yet the system itself often allowed less and less time for meaningful explanation.

Before the interview, I printed a page of prepared sound bites and added handwritten notes in the margins.

Anaïs smiled and called me a professional.

Apparently, no one else had arrived with prepared messaging.

I laughed a little at that moment, but privately I felt grateful.

Grateful for the guidance of fellow writers, speakers, and mentors who had encouraged me to prepare thoughtfully. Grateful for the writing journey itself, which has taught me that words matter most when they clarify rather than complicate.

The interview itself became less about pharmacy logistics and more about communication, trust, understanding, and patients navigating a healthcare system that often overwhelms them.

At one point, Anaïs asked me whether I believed pharmacy still had a bright future.

I told her I did — but only if healthcare becomes less fragmented and more collaborative.

Academia, retail pharmacy, hospitals, insurers, pharmaceutical companies, and policymakers cannot continue operating as disconnected silos with competing priorities. The future improves when those groups begin working together around a shared mission: helping patients safely navigate increasingly complex medications and healthcare systems.

Because the irony is impossible to ignore:

The more technologically advanced healthcare becomes, the more important human explanation becomes.

And yet that explanation is often the very thing modern systems leave the least amount of time for.

That realization sits at the heart of both Harmacy and my forthcoming book, Safe to Swallow.

The two projects approach the same problem from different directions.

Harmacy seeks to expose the growing pressures and realities inside the profession itself.

Safe to Swallow takes a patient-centered approach, helping readers better understand medications, risks, side effects, communication gaps, and the increasingly complicated world surrounding modern healthcare.

But both projects begin with the same recognition:

People do not simply need prescriptions.

They need understanding.

Near the close of the interview, I shared one final thought:

“The more complicated healthcare became, the more important human explanation became — precisely when the system allowed less time for it.”

When the cameras stopped rolling, I left feeling fulfilled, uplifted, inspired, and unexpectedly hopeful.

Not because I think healthcare’s challenges are simple.

They are not.

But because I was reminded that there are still people trying to improve the conversation with honesty, curiosity, and compassion.

And perhaps that is where meaningful change always begins.

What We Miss While Looking

Observation is not passive. It is one of the deepest forms of respect.

At conferences, airports, coffee shops, and even neighborhood sidewalks, I’ve noticed the same quiet phenomenon unfolding.

People are looking constantly.

At phones.
At screens.
At the schedules.
At each other.

Yet somehow, many things still go unseen.

A tired expression hidden behind professionalism.

A moment of hesitation before someone speaks.

The person standing quietly at the edge of the room, unsure how to enter the conversation.

The older man was eating lunch alone.

The overwhelmed parent is trying to hold everything together in the grocery store checkout line.

The colleague said “I’m fine” just a little too quickly.

Modern life rewards scanning.

We move quickly from headline to headline, task to task, notification to notification. We absorb information at a remarkable speed. We are connected to more people, more news, and more data than ever before.

And yet, I sometimes wonder if we are becoming less observant in the ways that matter most.

Because looking is not the same thing as noticing.

Noticing requires something more difficult.

It requires presence.

It asks us to pause long enough to recognize nuance, emotion, uncertainty, or humanity before moving on to the next thing demanding our attention.

I was reminded of this recently while attending several writing and speaking conference sessions. Beyond the presentations themselves, I found myself observing the quieter moments happening between them.

People search for confidence before asking a question.

Writers are wondering if their stories matter.

Professionals are trying to reinvent themselves after years in another career.

Conversations in hallways seemed far more meaningful than polished presentations on stage.

Many people arrive at conferences hoping not only to learn, but to feel seen. To feel understood. To feel less alone in their ambitions or uncertainties.

I suspect the same is true far beyond conferences.

Patients sitting in medical offices often want more than information. They want reassurance. Employees want acknowledgment. Friends want undivided attention, even for a brief moment.

Sometimes the most meaningful thing we offer another person is not advice, expertise, or solutions.

Sometimes it is simply our attention.

Not performative attention.

Real attention.

The kind that notices hesitation in a voice. Fatigue behind a smile. Excitement, someone is trying not to show too openly. Fear disguised as certainty.

Observation is often misunderstood as passive.

But I’ve come to believe it is one of the deepest forms of respect.

To notice another person fully, even briefly, is to quietly affirm their existence in a world increasingly designed to fragment attention.

Perhaps that is why certain conversations stay with us for years while others disappear almost immediately.

We remember the rare moments when someone truly listened.

In a culture increasingly trained to scan, sort, react, and move on, perhaps the rarest skill is still the simplest one:

to truly notice.

The Moment of Hesitation

There are moments at the pharmacy counter that never make it into the data.

No chart records them.
No insurance form captures them.
No prescription label explains them.

But they are there.

I remember a teenage boy — maybe fifteen or sixteen — standing at the counter. He had the look I had seen many times before. Not sick. Not in pain. But aware of himself in a way that only teenagers can be.

Acne.

Not severe, but enough. Enough to matter in hallways. Enough to shape how he walked into a room.

He handed me the prescription and waited.

I processed it. The system responded. I looked at the screen.

No insurance.

I told him the price.

And then it happened.

Not a refusal.
Not a complaint.
A hesitation.

It lasted maybe a second or two. But it was enough.

His eyes didn’t meet mine. His hand moved slightly, then stopped. He was doing the math — not just financial, but social, what this meant, what it cost. What would it say about him if he walked away without it?

“Does that mean they stopped the insurance?” he asked.

Not why is it so expensive?

Is there something cheaper?

But something quieter. More revealing.

He wasn’t asking about the medication.
He was asking about access. About coverage. About whether something had changed without him knowing.

About whether he still belonged inside the system.

That moment doesn’t show up in healthcare metrics.

But it should.

Because the hesitation is where everything is revealed:

understanding
confusion
vulnerability
trust

Or the absence of it.

We talk about adherence.
We talk about outcomes.
We talk about costs.

But before any of that, there is a moment.

A pause.
A calculation.
A quiet question.

Do I move forward?

Or do I step away?

In pharmacy, we have a name for what often follows when problems like this go unaddressed: a cascade. One issue leads to another. A small barrier becomes a larger one.

But sometimes, the cascade doesn’t begin with a drug.

It begins with hesitation.

And whether someone is there to notice it.

What We Celebrate — and What We Don’t Always See

Those things rarely make it into the story.

And yet, they’re the story.

Standing there, it occurred to me that what was being celebrated wasn’t just ten years of a company.

It was ten years of people continuing.

Continuing through change.
Continuing through doubt.
Continuing long enough to arrive at a moment like this.

We tend to celebrate outcomes.

But what often matters more… is what it took to get there.

It was a celebration.

A tenth anniversary, held in a craft whiskey distillery — warm light, conversation, and the easy rhythm of people who had come together to mark something that had lasted.

There was laughter.
Stories shared over hors d’oeuvres.
A sense of accomplishment that didn’t need to be announced.

I had a chance to speak with the founder, and later with several members of the team.
Nothing formal. Just conversation.

And yet, something about it stayed with me.

Ten years is a long time for a company.

Long enough to move past the early excitement.
Long enough to face uncertainty more than once.
Long enough to learn what works — and what doesn’t.

But we don’t always see that part.

We see the celebration.

We see the milestone.

We see the version that has endured.

What we don’t always see are the quieter moments that made it possible.

The decisions are made without certainty.
The stretches where progress wasn’t obvious.
The persistence required when things didn’t go as planned

The Problem Isn’t the Prescription

There’s a quiet assumption most of us carry into healthcare.

If something is prescribed, it must be the answer.

Not a piece of the answer.
Not part of a larger picture.
The answer.

Most of the time, we don’t question it.
We accept it.
We follow instructions.
We move on.

And when something changes — when a new symptom appears, or something doesn’t feel quite right — we assume something new has gone wrong.

So we look for another answer.

Another prescription.

But most problems don’t begin that way

In my experience, the problem rarely begins with a dramatic mistake.

It begins quietly.

A medication is added.
A symptom appears.
Another medication follows.

And somewhere along the way, no one steps back to ask:

What changed?

The part we don’t always see

Medications don’t exist in isolation.

They interact —
with your body,
with your conditions,
with each other.

Sometimes in predictable ways.
Sometimes in ways that are not.

But often in ways that go unnoticed… until something shifts.

Where a pause can matter

Not an expert.
Not training.

Just a pause.

A moment of consideration:

Is this new symptom actually new?
Could something I’m taking be contributing?
Has anything changed recently?

Simple questions.

But not always easy ones — especially when things feel rushed, uncertain, or unclear.

Because healthcare is human, too

Patients are trying to understand what’s happening to them.
Providers are managing time, information, and competing demands.

And when those two realities meet, something can get lost.

Not care.

But clarity.

Questions feel like interruptions.
Answers feel incomplete.

And when clarity is missing, assumptions quietly take its place.

So we move forward — without connecting the dots

We assume:

the new symptom is unrelated
the new prescription is necessary
someone else has already considered the whole picture

And sometimes, that’s true.

But not always.

A small shift

Instead of asking:

What’s wrong with me?

There’s another question that can open things up:

What changed?

It doesn’t solve everything.

But it has a way of slowing things down…
of reconnecting pieces that might otherwise remain separate.

Because the problem isn’t always the prescription

Sometimes, it’s the absence of connection.

Between symptoms and causes.
Between questions and answers.
Between what is happening… and what is understood.

And not just in healthcare

We do this in life, too.

We react before we reflect.
We accept what’s presented without asking what led to it.
We assume understanding where there may be none.

And often, the result is the same:

We move forward — without clarity.

A final thought

Most of the time, the problem isn’t what we think it is.

But we don’t always pause long enough to notice.

The Dog That Didn’t Bark

We are trained to notice what happens.

The raised voice.
The sudden change.
The obvious signal that something is wrong.

We look for movement.
For noise.
For disruption.

And when we don’t see it, we assume nothing is there.

But sometimes, the most important clue…

is what never happened.

There’s an old mystery story involving a dog.

A crime takes place in the night.
Someone approaches, something unfolds.

And yet, the dog remains silent.

No barking.
No warning.
No sign that anything unusual occurred.

At first, this seems unremarkable.

Until someone asks a different question:

Why didn’t the dog react?

Because a dog reacts to strangers.
To threats.
To the unfamiliar.

Silence, in this case, wasn’t absence.

It was information.

The dog wasn’t alarmed… because it recognized who was there.

What didn’t happen revealed everything.

We don’t often think this way in our own lives.

We look for what is said, not what is withheld.
We notice what appears, not what is missing.

But absence can be its own form of presence.

The call that never came.
The question that no one asked.
The reaction that didn’t occur.

These are easy to overlook.

But they are often the quiet signals worth paying attention to.

Sometimes, the truth hides not in plain sight.

It’s hidden in what never showed up at all.

What didn’t happen… might be the most important thing to notice.

We Pick Up Where We Left Off

There are conversations we mean to return to.

A message we intend to answer.
A lunch we mean to schedule.
A person we think of, briefly, and then again later.

And then time passes.

Not dramatically.
Not with intention.
Just enough for us to wonder if the moment has passed.

If the opportunity has closed.

If the silence has said something we didn’t mean to say.

I had one of those recently.

A note that sat longer than it should have.
An invitation to reconnect that I didn’t act on right away.

And then, eventually, I reached out.

Not with urgency.
Not with an explanation.

Just a simple message.

What came back was not hesitation.

It was openness.

A willingness to meet.
A quiet generosity.
A sense that the conversation was not lost — only paused.

We met for lunch.

There was no need to revisit the delay.
No need to account for time.

We picked up where we left off.

And it reminded me of something easy to forget:

Not all missed moments are missed.

Some are waiting.

Waiting for the right day.
The right energy.
The right version of us to arrive.

We often think of time as something that closes doors.

But sometimes, it does the opposite.

Some conversations don’t end.
They wait for us to return.
